By using our Services or providing your personal information to us, you consent to the practices described in this Privacy Notice on your behalf and on behalf of the person (alive or deceased) whose information you have shared with us.

The collection, use, and disclosure of your information will always be consistent with our mission to reduce the suicide rate in the military community and save lives.

Information We Collect

We may collect or receive the following information from or about you:

contact information that identifies you, such as your name, phone number, and postal address when you apply for our Services;
payment information, such as payment method, or payment account number, and payment authorization when you make a purchase or donate;
electronic contact information, such as email address and mobile phone number;
demographic and sensitive information, such as your gender, race or ethnicity, marital status, geographic location, and/or primary language;
social media identifiers and/or information concerning your interests or opinions expressed through social media postings;
limited health and care service history information to confirm eligibility for our Services and to assess appropriate care when requesting clinical information or clinical services;
technical usage information using tracking technologies (for example, cookies, tags, pixels, web beacons, and other similar technologies (collectively, “Tracking Technologies”)), such as IP address, browser type, unique device identifier, geolocation, and device operating system, which may be linked to you;
limited health insurance or plan information, which may include provider or plan name, policy number, scope of coverage, and exclusions; and
any other personal information you voluntarily provide to us when engaging with us.

How We Use Your Information

We will use your information to provide our Services consistent with our mission to reduce the suicide rate in the military community and save lives. This includes, where applicable:

processing donations and payments, and providing you receipts;
communicating with you about our mission, events, and Services, as well as information you request from us or our third-party partners;
responding to your inquiries and feedback;
sending you newsletters and updates you have elected to receive;
processing your volunteer application and managing your volunteer activities;
communicating with you, posting your comments, or establishing a social connection with you on social media platforms;
measuring engagement and the effectiveness of our campaigns, and determining areas of interest of our constituency;
processing your application and enrollment in our Black Box Project, including conducting analytics on digital devices as part of the Back Box Project;
determining eligibility for and facilitating the provision of treatment services to you;
monitoring and confirming the provision of clinical services provided by clinicians with your authorization;
connecting you to programs or services you have requested, and providing program follow-up and evaluation;
managing our operations, such as conducting audits, fraud monitoring and prevention, data analysis, recordkeeping, developing new services, enhancing, improving, or modifying our programs and services, identifying usage trends, determining the effectiveness of fundraising campaigns and mission activities, and updating and expanding our Services;
complying with applicable laws, rules, and regulations, and responding to law enforcement inquiries and requests for your information; and
with your consent, for other purposes you expressly authorize us to use your information at the time of collection.

Disclosure of Information

We may disclose your information in connection with the purposes noted above in the following ways:

to our employees, contractors, and third-party service providers who need to know such information to perform their duties such as processing donations, sending emails, fulfilling orders, rendering services, and analyzing data;
to send you marketing and promotional communications and materials concerning our Services and other products, services, and programs from us or third-parties we think may be of interest to you;
with third-party marketing partners and providers in connection with marketing analytics, targeted advertising, and cross-context marketing;
with treatment providers that participate in our program for the provision of treatment;
to coordinate treatment-related services with a referring organization;
if required by law, court order, or government regulations;
to enforce our Terms of Use;
to defend and protect our rights, privacy, safety, or property, and/or that of our affiliates, you, or others;
under circumstances we believe appropriate to coordinate treatment-related services with your provider or health plan, or for permissible public health purposes;
with any reorganization, merger, combination, transfer or winding down of our operations or assets; and
for research purposes.

We do not sell, rent, or share your personally identifying information to third-parties for their marketing purposes.

Usage Information Collected

In addition to information that uniquely identifies you when you use our digital Services, we also use Tracking Technologies to collect non-identifying standard web server traffic patterns and usage information, such as IP address, browser type, unique device identifier, geolocation, device operating system, website and application usage, browser information, navigation activities, preference settings, and length of website visit information (“Usage Information”). We collect and use Usage Information automatically to measure the success of our marketing campaigns, and compile statistics about digital service usage, conversion, and response rates. We also work with third-party service providers to target advertisements and outreach based on this information, in some cases combining data we collect using Tracking Technologies with other de-identified data.

In addition, we may allow third-party advertisers and their service providers to display content-specific advertisements to you on our digital Services or after you have left our digital Services, such as when you go to other websites. These third-parties may use Tracking Technologies to tailor advertisements to you based on interests shown when browsing our digital Services or by visiting other digital platforms. We do not control the third-party advertisers’ Tracking Technologies or the data collected by them. The use of such Tracking Technologies for advertisements by these third-parties are not covered by this Privacy Notice. We encourage you to review such third-party’s privacy policies and other terms to understand the choices you may have concerning information collected from or about you by these third-parties.

Certain Tracking Technologies may be rejected, disabled, or removed by tools that are available as part of most commercial browsers, and in some but not all instances can be blocked in the future by selecting certain settings. Each browser you use will need to be set separately and different browsers offer different functionality and options in this regard. Also, please be aware that if you disable or remove these Tracking Technologies some parts of our digital Services may not work and when you revisit our digital Services your ability to limit browser-based Tracking Technologies is subject to your browser settings and limitations. Various third parties are developing or have developed signals or other mechanisms known as “do not track signals”. For instance, some browsers may have such signal options, although they differ as to if the signal is set as default or must be turned on by the user. Further, there is currently no standard as to what sites and app operators should do with regard to these signals. Currently, we do not monitor or take any action with respect to these signals or other mechanisms, although we may consider doing so if an understandable and practical standard is developed and widely adopted.

For analytics , we use Google Analytics, a web analytics service provided by Google, Inc. (“Google”). Google Analytics may set cookies on your browser or mobile device or read cookies that are already there to collect information. Google Analytics may also receive information about you from apps you have downloaded that partner with Google. Google Analytics collects information such as how often you use our digital Services. We use the information provided by Google Analytics to improve these Services. We do not combine the information collected through the use of Google Analytics with personally identifiable information.
